Clinical Social Worker

Position Overview:The Hospital Social Worker provides psychosocial support to patients and their families, helping them cope with the emotional, financial, and social challenges associated with illness, hospitalization, and ongoing care. The role involves patient advocacy, counseling, care coordination, and connecting patients with internal and external resources to ensure holistic support throughout the continuum of care.
Key Accountabilties:Patient Support & Counseling Provide emotional support and counseling to patients and families to help them manage stress, grief, trauma, or adjustment to illness and hospitalization. Assist patients in understanding medical conditions, treatment options, and care plans in collaboration with the healthcare team. Facilitate family meetings to support decision-making and discharge planning.
Care Coordination & Discharge Planning Collaborate with physicians, nurses, and other healthcare providers to ensure smooth discharge planning. Arrange post-discharge services such as rehabilitation, home care, community resources, and follow-up appointments. Support patients and families in navigating the healthcare system and accessing financial or community resources.
Advocacy & Resource Linkage Advocate for patient rights and equitable access to healthcare and social services. Connect patients with government agencies, NGOs, charities, and community resources for financial, housing, or social support. Educate patients and families on available hospital and community programs.
Crisis Intervention Provide immediate support and intervention during critical situations such as domestic violence, child abuse, mental health crises, or end-of-life care. Work with relevant authorities to ensure patient safety and wellbeing.
Education & Qualification DHA license or eligibility as Clinical Social Work BSc in Social Work And Master’s degree in Social Work
Experience Minimum 3-5 years of relevant experience in a related field. Experience in counseling, crisis intervention, and resource coordination is preferred.
