1. Support Social Work Activities
- Assist the Social Worker in delivering psychosocial support services and follow-up with patients and families.
2. Preliminary Assessments
- Conduct initial client interviews or gather demographic/social information for assessment purposes under supervision.
3. Client Assistance
- Help patients complete necessary forms, service applications, or paperwork for referrals and resource programs.
4. Coordinate Community Services
- Research and coordinate available community resources, support services, and relevant agency contacts.
5. Documentation & Records
- Maintain case files, update logs, document client interactions, and assist with data entry.
6. Communication & Liaison
- Facilitate communication between patients, families, clinical social workers, and hospital staff.
7. Emotional Support
- Provide general emotional support and reassurance to patients and families within the scope permitted (not therapeutic counseling unless trained/licensed).

Qualifications:
- Bachelor’s or Master’s degree in Social Work (BSW or MSW)
- Valid state licensure/certification (as required by location)
- Knowledge of social service systems and community resources
- Strong communication and interpersonal skills
- Ability to handle sensitive and confidential information
