1.Conduct comprehensive social assessments to evaluate the social, economic, and environmental conditions of patients and provide appropriate recommendations and social services.
2.Prepare and maintain patients’ social records, collect and document relevant information about patients and their family circumstances to support the development of treatment plans.
3.Follow up regularly with patients and maintain social communication with them according to the nature of each case.
4.Conduct home visits when required in accordance with applicable policies and procedures.
5.Assist in securing necessary medical supplies for patients facing social or financial challenges and facilitate referrals to appropriate charitable or social organizations when required.
6.Prepare periodic statistical reports on the social services provided to patients.
7.Organize and coordinate social, cultural, religious, and awareness activities for patients.
8.Provide psychosocial support to patients and their families to help reduce stress and anxiety.
9.Plan and implement educational and awareness programs for patients and their families to promote health and social awareness.
10.Participate in patient-related committees and contribute to the activation of international medical and social awareness events.
11.Prepare social reports and certificates that support patients and their families in obtaining relevant assistance or services.
12.Facilitate patient processes with other hospitals by coordinating with social services departments in external healthcare facilities.
13.Perform any other duties related to the nature of the job as assigned.
5-7 years (Relevant experience in social work, preferably within healthcare or hospital settings)
CDA License required
