<p><span>The Pharmacist provides comprehensive pharmaceutical care in the community setting that assists patients to achieve optimum health.</span></p><br>
<p><span>The Pharmacist is responsible for the provision and management of effective pharmaceutical care to individuals and/or families in the community or in Patient Support Programs. <p><span>Based on the agreed-upon services and workflow, the Pharmacist provides skilled pharmaceutical services in accordance. Duties include but are not limited to:</span></p><br>

<li><span>Completes pharmacy operational requirements by organizing and directing technicians’ workflow, verifying their preparation and labelling of pharmaceuticals, and verifying order entries, charges, and inspections.</span></li>
<li><span>Provides pharmacological information by answering questions and requests of health care professionals and counselling patients on drug therapies.</span></li>
<li><span>Develops staff’s pharmacological knowledge by participating in programs and training pharmacy staff, students, interns, externs, residents, and health care professionals.</span></li>

The role involves close collaboration with physicians, nurses, and multidisciplinary healthcare teams to develop, monitor, and review patient-specific treatment plans while promoting evidence-based practice and medication safety.<br><br>Responsibilities:<br><ul> <li>Supervise junior staff and ensure their abidance to going rules and policies as applied and implemented in the section/department/organization, as per requirement.</li> <li>Participate in department/organization/regulatory bodies’ clinical projects.</li> <li>Optimize patient care in defined wards/units by planning, managing, monitoring, advising and reviewing specialist pharmaceutical care programs for patients.</li> <li>Undertake a medication review/ history of newly admitted patients to identify problems or medication issues that may influence the patients’ treatment plan, including simplifying complex regimes for patients in designated wards.</li> <li>Formulate treatment plans for inpatients, considering their presenting symptoms, medical history (including previous responses to medication), and the evidence base for pharmacological approaches in that condition. These treatment plans can be presented at multidisciplinary meetings for discussion and review.</li> <li>Discuss future treatment options with patients /carers.
