The Registered Nurse will work as Patient Care Coach to provide compassionate and empathetic guidance to help patients to manage the impact of their condition and to receive their treatment effectively. The Registered Nurse / Care Coach is a qualified healthcare professional who complements the education and care provided by the patient’s own healthcare team. The role involves regular patient contact, in person and by telephone, and is responsible for building rapport and helping to address their patients’ personal challenges and needs. Facilitate successful communication between patients, physicians and third party payers that leads to shared motivation to achieve treatment goals. Ensure continuous patient education, motivation, consultation and follow up. Responsibilities Introduce to physicians the role of the Patient Care Coach (PCC) and their purpose to assist them with their patients. Have a consent form signed by the physician to allow the PCC to contact his/her patients for education, follow ups and collection of health information. Have a consent form signed by the patient and acknowledged by the treating physician to allow the PCC to teach him/her self-injection, follow up on his general health status, treatment outcomes and adverse events (if spontaneously reported), collect relevant health information and update the treating physician periodically. Accurately capture patient details following face to face based and telephone interactions and assessments in the programme CRM system. This would include scheduled contacts as well as any ad hoc inbound queries from patients. Ensure patients data privacy is 100% protected and confidential information is not disclosed under any circumstances. Ensure regular update for each physician individually regarding their patients’ status: The update would include general condition of the patient, improvement, side effects if any, compliance, emotional status, patients concerns, etc. Visit the physicians regularly (as per project plan) to present updates on their patients. Educate patients/parents/family members about the disease. To assess and provide tailored interventional support to help patients overcome potential psychological or behavioural challenges that may impact their ability to manage their treatment or specific disease. Educate patients and their care givers to correctly and confidently use their prescribed treatment medication. Manage and report any potential adverse events, product quality issues or other perceived safety risks according to defined program procedures. Give comprehensive instructions to the patients about detecting and reporting side effects. Gather all the detailed data from the patient and report promptly to as per local Pharmacovigilance SOPs. Educate patients about proper storage and administration of their treatment medication. Be available for and follow up regularly on questions and queries. Highlight the importance of regular check-ups and doctor’s visits during the course of the treatment as ordered by the physician. Provide emotional support to patient and family. Drive adherence to therapy to optimize treatment outcomes. Advise patients not to take any decision on discontinuation of therapy or dose changes without consulting their treating physicians. Give the patient the instructions for Third Party Payer procedures and assist when needed (whenever applicable). Remind patients of prescription refill dates & doctor’s appointments. Provide injections and nursing care to patients, as when required. Submit to the direct manager aggregate data on patients’ adherence on monthly basis or whenever needed. Requirements Education: Bachelor’s Degree in Nursing holding valid licensure in Kuwait. Experience: At least 2 years of experience in health education or a similar role. About Les Toises Les Toises – Centre de psychiatrie et psychothérapie is a Swiss private outpatient mental health group founded in Lausanne. نبذة عن Les Toises تعد Les Toises – مركز الطب النفسي والعلاج النفسي مجموعة سويسرية خاصة للرعاية الصحية النفسية الخارجية تأسست في لوزان. ما بدأ كمركز واحد نما ليصبح 9 مؤسسات: 8 في سويسرا وواحدة في الإمارات العربية المتحدة؛ جميعها مبنية حول فكرة واحدة: أن الرعاية النفسية والعلاجية المتخصصة يجب أن تُقدم وفقاً لأعلى المعايير السريرية، وأن يتم تصميمها لتناسب الفرد بدلاً من التركيز على التشخيص فقط. يمثل عيادتنا الجديدة في دبي الخطوة التالية: أول مؤسسة سريرية متكاملة لنا خارج سويسرا. تجلب عيادة دبي نموذج Les Toises — متعدد التخصصات، قائم على الأدلة، ومخصص لكل مريض — إلى مدينة تتميز عائلاتها بأنها دولية ومتنقلة وكثيراً ما تبحث عن رعاية الصحة النفسية عبر عدة دول وأنظمة. نحن نقوم ببناء الفريق الآن، وسيساهم الأشخاص الذين ينضمون في هذه المرحلة في تشكيل ممارسات العيادة.الدور نحن نبحث عن أخصائي نفساني لديه تركيز سريري على الأطفال والمراهقين للانضمام إلى الفريق السريري التأسيسي. الدور متاح بدوام كامل أو جزئي. يسعدنا مناقشة ساعات العمل المخفضة، أو عدد ثابت من الجلسات السريرية أسبوعياً، أو ترتيب يتناسب مع ممارسة قائمة بالفعل. تعد الخبرة في العمل مع العائلات وأنظمة الآباء والأبناء ميزة كبيرة: فجزء كبير من العمل الذي نقوم به مع الشباب يتم بوجود العائلة في الغرفة.ما ستفعله <ul><li>تقديم العلاج النفسي الفردي للأطفال والمراهقين عبر مجموعة من الحالات — القلق، اضطرابات المزاج، الصدمات، صعوبات النمو العصبي، تنظيم السلوك والعواطف، والضيق المتعلق بالمدرسة.</li><li>إجراء التقييمات السريرية والصياغات، والمساهمة في التقييمات النفسية والتربوية النفسية عند الحاجة.</li><li>العمل مع الآباء والعائلات، بما في ذلك التوجيه الأبوي، والجلسات العائلية، والتدخلات النظامية.</li><li>التعاون الوثيق مع الأطباء النفسيين والزملاء في مناقشات الحالات متعددة التخصصات.</li><li>التواصل، بعد الحصول على الموافقة، مع المدارس وأطباء الأطفال وغيرهم من المهنيين المشاركين في رعاية الشاب.</li><li>الحفاظ على السجلات السريرية وفقاً لمعايير Les Toises والمساهمة في عمليات الجودة والحماية في العيادة.</li><li>المشاركة في برنامج التدريس والإشراف والتعليم المستمر في العيادة.</li></ul>ما نبحث عنه أساسيات <ul><li>درجة الماجستير أو الدكتوراه في علم النفس السريري، أو مؤهل معادل ومعترف به.</li><li>تدريب متخصص بعد التخرج وخبرة سريرية يمكن إثباتها في علم نفس الأطفال والمراهقين.</li><li>ترخيص هيئة الصحة بدبي (DHA) كأخصائي نفساني، أو الأهلية والاستعداد لإكمال ترخيص هيئة الصحة بدبي (نحن ندعم المرشحين خلال هذه العملية).</li><li>أساس قوي في نموذج واحد على الأقل قائم على الأدلة مطبق على الشباب — العلاج السلوكي المعرفي (CBT)، العلاج النظامي/العائلي، تقنية EMDR، العلاج بالقبول والالتزام (ACT)، أو العلاج النفسي الديناميكي للأطفال.</li><li>إجادة اللغة الإنجليزية تحدثاً وكتابة.</li><li>الارتياح في العمل مع مجموعة مرضى دولية ومتعددة الثقافات.</li></ul> مرغوب فيه <ul><li>خبرة في العمل مع العائلات وعمل الآباء والأبناء، بما في ذلك التدريب على العلاج النظامي أو العائلي.</li><li>لغات إضافية — تعتبر العربية و/أو الفرنسية ذات قيمة خاصة لقاعدة مرضانا.</li><li>خبرة في التقييم الموحد (اضطراب فرط الحركة ونقص الانتباه ADHD، اضطراب طيف التوحد ASD، البطاريات التربوية النفسية).</li><li>خبرة سابقة في عيادة خاصة أو متعددة التخصصات.</li><li>اهتمام بالتدريس أو الإشراف أو البحث السريري.</li></ul>
