<span>We are seeking compassionate and dedicated Home Care Nurses with a valid DHA, DOH, or SHA License/Eligibility to provide high-quality nursing care to patients in their homes.<br> The successful candidate will deliver patient-centered care, monitor health conditions, administer treatments and medications, educate patients and families, and ensure continuity of care while maintaining the highest standards of clinical practice.<br> Key Responsibilities Provide professional nursing care to patients in home care settings.<br> Assess patients' physical, psychological, and social needs and develop individualized care plans.<br> Administer medications, injections, intravenous therapy, and prescribed treatments as per physician orders.<br> Monitor and record vital signs, patient progress, and response to treatment.<br> Perform wound care, catheter care, tracheostomy care, PEG feeding, suctioning, and other nursing procedures as required.<br> Assist patients with activities of daily living (ADLs), including personal hygiene, mobility, feeding, and rehabilitation support.<br> Educate patients and family members on disease management, medication compliance, nutrition, and preventive healthcare.<br> Maintain accurate nursing documentation and patient records.<br> Recognize changes in patient conditions and promptly communicate with physicians and healthcare teams.<br> Adhere to infection prevention and control protocols and patient safety standards.<br> Ensure compliance with DHA, DOH, or SHA regulations and home healthcare policies.<br> Maintain patient confidentiality and deliver compassionate, culturally sensitive care.<br> Qualifications & Requirements Bachelor's Degree or Diploma in Nursing from a recognized institution.<br> Valid Registered Nurse (RN) license in the home country.<br> Valid DHA, DOH, or SHA License/Eligibility is mandatory.<br> Minimum 2 years of clinical nursing experience , preferably in home healthcare, medical-surgical, ICU, emergency, or rehabilitation settings.<br> Current BLS certification ; ACLS certification is an added advantage.<br> Strong clinical assessment and patient care skills.<br> Excellent communication, interpersonal, and documentation skills.<br> Ability to work independently and travel to patients' residences.<br> Compassionate, reliable, and committed to delivering high-quality home healthcare.<br></span> </div>
