<h2 class="h5">Job description</h2> <div class="t-break" data-jb-field="description"> <div dir="rtl"><p><span><span>متابعة وتقييم دراسات الحالات وفقاً للإجراءات والمعايير المعتمدة</span><span>لضمان سير خطط التدخل والتأكد من مدى تحقيق الأهداف لإغلاق الحالات بالشكل الصحيح.</span></span></p><br><p><span><b><span><strong>الواجبات والمسؤوليات التشغيلية</strong></span></b></span></p><br><ol><li><p><span><span>مراجعة نتائج خطط التدخل واجراء التعديلات على الخطط إن لزم الأمر بناء على التقدم في سير العمل ومدى تحقيق المستهدفات من تقديم الخدمات.</span></span></p><br></li><li><p><span><span>اعتماد إغلاق الحالات بعد التأكد من تنفيذ الخطط على أساس المعايير والأطر المعتمدة وبناءً على تقييم تحقيق المستهدفات ومراجعة التقارير النهائية للحالات وتقديم الملاحظات والتوصيات.</span></span></p><br></li><li><p><span><span>إجراء اجتماعات "مراجعات فريق العمل" والتحقق من تقييمات القبول المبدئية، ودراسة الحالات وخطط التدخل وتقارير سير العمل، وإغلاق الحالات لضمان الجودة والاتساق.</span></span></p><br></li><li><p><span><span>التفواض مع مقدمي الخدمات الخارجيين لضمان تنفيذ أدوارهم في خطط التدخل والتأكد من أن الخدمات المقدمة لمتعاملي الهيئة في مستوى الجودة المتفق عليه حسب اتفاقيات تقديم الخدمات.</span></span></p><br></li><li><p><span><span>مراجعة واعتماد تقييمات المخاطر لخطط التدخل المعدة من قبل الأخصائيين والمتابعة الدورية للتأكد من وضع تلك المخاطر للتدخل الاستباقي من خلال الخطط التصحيحية بما يضمن تحقيق المستهدفات.</span></span></p><br></li><li><p><span><span>التنسيق مع مقدمي الخدمات الداخليين (إدارات وأقسام الهيئة ) (مثل: كبار المواطنين؛ أصحاب الهمم؛ الاستشارات الأسرية؛ حماية الطفل؛ المنافع والتمكين المالي) والمتابعة لضمان قيامها بادوارها حسب خطط التدخل ومناقشة التعديلات على الخطط إن لزم الامر.</span></span></p><br></li><li><p><span><span>متابعة تقديم خدمات الهيئة للمتعاملين المحولين من الجهات الخارجية الأخرى المشاركة في النظام الموحد لإدارة الحالات الاجتماعية والانسانية.</span></span></p><br></li><li><p><span><span>تنظيم اجتماعات دورية</span><span>مع الاخصائيين المعنيين بالحالات والمتعاملين لتلقي ملاحظات المتعاملين ومقدمي الخدمات؛ لمراجعة</span><span>فعالية الخدمات وجودتها.</span></span></p><br></li><li><p><span><span>عمل زيارات ميدانية لمتابعة الحالات وفقاً للمعايير المعتمدة لمتابعة الحالات الطارئة والعالية الخطورة وتقييمها.</span></span></p><br></li><li><p><span><span>متابعة نظام الشكاوى عن الحالات التي يتم إدارتها وإجراء المراجعات الدورية لمسار رسم الإجراءات الخاصة بإدارة الحالات سواء مع الجهات الخارجية المقدمة للخدمات أو مع الإدارات داخل الهيئة ورفع توصيات التحسين والتطوير للرئيس المباشر.</span></span></p><br></li><li><p><span><span>إعداد التقارير الدورية عن مدى الانجاز في خطط العمل ودراسات الحالات ورفعها للاعتماد.</span></span></p><br></li><li><p><span><span>توثيق المراجعات على الحالات وخطط التدخل بشكل دوري على قواعد البيانات الالكترونية لدعم المتابعة الفعالة والتحديث بشكل مستمر.</span></span></p><br></li><li><p><span><span>وضع أنظمة للمتابعة والرقابة على تنفيذ النظام الموحد لإدارة الحالات وتحديد الفجوات ومجالات التحسين ورفع التوصيات إلى مدير الإدارة.</span></span></p><br></li><li><p><span><span>ضمان الامتثال للتشريعات الاجراءات ومعايير النظام الموحد لإدارة الحالات.</span></span></p><br></li><li><p><span><span>تقديم الدعم لاختيار أخصائيي إدارة الحالات وفقاً للتخصصات.</span></span></p><br></li><li><p><span><span>الإلتزام بسياسات وإجراءات أمن المعلومات المعتمدة بالهيئة والعمل على الامتثال بالضوابط لضمان سرية المعلومات</span></span></p><br></li><li><p><span><span>أية</span><span> مهام واختصاصات أخرى في نطاق مسئوليات الوظيفة.</span></span></p><br></li></ol> <p><span><b><span><strong>المتطلبات التعليمية</strong></span></b></span></p><br><p><span><span>·</span></span><span><span>بكالوريوس في الخدمة الاجتماعية أو علم النفس أو ما يعادلهما.</span></span></p><br><p><span><span>·</span></span><span><span>اجادة اللغتين العربية والانجليزية (محادثة وكتابة).</span></span></p><br><p><span><b><span><strong>متطلبات الخبرة</strong></span></b></span></p><br><p><span><span>·</span></span><span><span>7 سنوات خبرة على الأقل في مجال ذو علاقة.</span></span></p><br></div> </div>
